A fully-insured health plan is the traditional way to structure an employer-sponsored health plan. With a fully-insured plan, the company is responsible for paying the premium to the insurance carrier. In general, premiums are standard for a year and are based on the number of employees enrolled in the plan. The insurance carrier collects premiums and then pays the claims based on the group's benefits. The employees and their dependents are required to pay any deductible amounts, copays, or coinsurance required under the benefit policy.
